Gastrik biyopsi örneklerinden Helicobacter pylori tanısında "Nested" polimeraz zincirleme reaksiyonunun değeri

Polimeraz zincirleme reaksiyonu (PCR), gastrik biyopsi örneklerinden Helicobacter pylori'nin özgül tanısı için hızlı, duyarlı ve doğru bir yöntemdir. Çalışmamızda, laboratuarımıza gönderilen 104 gastrik biyopsi örneği kültür, Giemsa boyama, PCR ve nested PCR yöntemi ile H.pylori yönünden araştırılmıştır. Biyopsi örneklerinin 35'inde (%33) kültürden H.pylori izole edilirken, Giemsa boyama ile 25'inde (%24), histopatolojik inceleme ile 36'sında (%34), PCR ile 38'inde (%36) ve nested PCR ile 43'ünde (%41) pozitiflik belirlenmiştir. En az üç yöntemin pozitifliğine göre; nested PCR yönteminin duyarlılığı %100, özgüllüğü %91, pozitiflik tahmini değeri %86, negatiflik tahmini değeri ise %100 olarak bulunmuş ve bu yöntemin H.pylori tanısında değerli bir referans yöntem olabileceği düşünülmüştür. Polymerase chain reaction (PCR) is a rapid, sensitive and accurate method for the specific detection of Helicobacter pylori from gastric biopsy specimens. In our study, 104 gastric biopsy specimens submitted to our laboratory were investigated by culture, Giemsa staining, PCR and nested PCR methods for the determination of H.pylori. H.pylori was isolated from 35 of the samples (33%) in culture, whereas Giemsa staining was positive in 25 (24%), histopathologic examination was positive in 36 (34%), PCR was positive in 38 (36%) and nested PCR was positive in 43 (41%) of biopsy specimens. The sensitivity, specificity, positive and negative predictive values of nested PCR according to the positive results of the other three tests were found to be 100%, 91%, 86% and 100%, respectively.
