Continuity is also a pillar of the Patient's Medical Home (PMH) model developed by the College of Family Physicians of Canada, yet it can be challenging to achieve in academic teaching environments. They review the concepts of continuity in patient care and continuity in education, and the challenges of their execution in an academic setting. They also discuss the opportunities within and outside the PMH model to bring together continuity for both patient care and resident education. |
