Zusammenfassung:This session will report on early findings from an innovative partnership developed to bridge the gap in access to palliative care. We brought together an insurance company, medical groups, hospice organizations, patients/caregivers, and a research team to design, implement, and evaluate in Accountable Care Organizations (ACOs) an evidence-based Home-based Palliative Care (HBPC) program that originally launched within a managed care environment. This session presents on the process of identifying and implementing a HBPC model, engaging key stakeholders, and lessons learned from this process. We will discuss how to assess readiness for palliative care programs among ACO leadership and practice and among community-based hospice organizations. Additionally, we will present effective engagement strategies for medical groups, primary care providers, and patients. We also will present on novel administrative and contracting relationships required in the design of these new services.
