Zusammenfassung:The purpose of the Patient-Centered Medical Home (PCMH) in the United States is to position the primary care system to provide better health care, better health, and lower cost per capita coupled with high-quality, accessible, and efficient care for all Americans. The PCMH is a promising model for transforming the organization and delivery of primary care. This presentation hightlights the published White Paper entitled “Patient-Centered Medical Homes and the Care of Older Adults” in the fall of 2016. It provides a roadmap for the PCMH to continue the transformation of primary care delivery of care to the spectrum of older patients and/or their caregivers. The presentation will discuss the approach needed to recognize, facilitate, and encourage improved care for older adults in a PCMH setting by providing person-oriented and patient-goal directed health care that is inclusive of connections to community-based organizations.
