Zusammenfassung: | From August 2021 to October 2022, select patients 65 years or older with serious chronic illness (e.g., cancer, chronic lung disease, chronic kidney disease, dementia, heart failure, ischemic coronary artery disease, cirrhosis, diabetes mellitus with complications, history of cerebrovascular event, or osteoporotic fracture) and a frailty index score of ≥0.25 (based on the deficit accumulation frailty index) (5) were identified at the Beth Israel Deaconess Medical Center geriatrics outpatient clinic. Diamonds indicate changes in Physical Function PROMIS score of greater than five from baseline that can be explained by either lifestyle changes including exercise, diet change, and physical therapy (blue diamonds), or an acute medical event including Emergency Department admission, hospitalization, fall, cognitive decline or other injuries (red diamonds). PROMIS PF identified acute changes in some patients that led to early interventions, which suggests utility in high-risk patient monitoring. Because this was at a single institution with a small sample of predominantly white race, our findings may not be generalizable. |
