Delayed/failed diagnosis of meningitis 5 Delayed diagnosis of slipped upper femoral epiphysis 5 Delayed diagnosis of a brain tumour 3 Delayed diagnosis of pulmonary TB that led to TB meningitis 2 Delayed/failed diagnosis of meningococcal septicaemia 2 Delayed diagnosis of testicular torsion 2 Miscellaneous cases 23 The miscellaneous cases included a delayed diagnosis of appendicitis, a delayed diagnosis of neonatal jaundice that lead to kernicterus, a delayed diagnosis of septic arthritis, failure to follow-up a urine result in a case of a urinary tract infection with subsequent scarring, a medication error and some other rarer conditions. |
