Zusammenfassung:	In our opinion, laparoscopic Heller myotomy is an optimal treatment for achalasia with extreme megaesophagus, relieving the obstruction of the esophagus and the compression of mediastinal strictures. [...] we believe that laparoscopic Heller myotomy should be the first option for the treatment of severe achalasia, and esophagectomy should be reserved only for patients who already underwent a myotomy without relief of symptoms.
