Zusammenfassung:The Visiting Nurse Association of Cleveland developed a mental health assessment form which facilitates nurses' assessment of patients' status, encourages the setting of realistic goals for patients, and makes visible slight changes in behavior. It is also expected that it will improve the quality of the VNA's periodic evaluations of its entire mental health program.
