Zusammenfassung: | Prognosis is poor with a median survival of 4 months in patients presenting with heart failure. 1 The echocardiograph often shows a thickened left ventricle with a granular speckled appearance due to myocardial infiltration. Late gadolinium imaging often shows a characteristic pattern of global subendocardial enhancement which is thought to reflect increased amyloid deposition in the endocardium. 2 Chemotherapy targeting clonal plasma cells can reduce circulating free light chains and improve survival in patients with systemic AL amyloidosis. 3 Assays measuring serum free light chain levels can be used to monitor treatment. |
