Prediction of Awakening after Cardiac Arrest

To the Editor: With reference to the article by Longstreth et al. on awakening after out-of hospital cardiac arrest (June 9 issue), 1 the clinician should keep in mind the ability of a high-dose dopamine infusion to cause dilated, nonreactive pupils. 2 This observation may have clinical importance, since many patients who have an out-of-hospital cardiac arrest require dopamine early in their resuscitation and the pupillary response is a heavily weighted factor in the application of Longstreth el al.'s scores.
