Zusammenfassung:	Cross-training nursing personnel to provide follow-up home care to patients with congestive heart failure has resulted in a dramatic drop in readmissions. Significant reduction in length of stay and cost for readmissions has prompted this hospital to expand this type of service to other patient populations. An added benefit is high job satisfaction among nurses.
