Zusammenfassung:	This report describes surgical treatment consisting of mitral valve repair and septal myectomy in a young patient with a combination of hypertrophic cardiomyopathy and intrinsic mitral valve disease. The posterior mitral valve leaflet was elongated and was subject to systolic anterior motion, creating left ventricular outflow tract obstruction. With the described treatment, it was possible to avoid mitral valve replacement in this patient.
