A Couples Advance Directives Interview Using the Five Wishes Questionnaire

This article provides couples an opportunity to consider personal wishes for end-of-life decision making. There is brief overview of some of the issues and organizations addressing advanced directives. A portion of the Five Wishes1 questionnaire, presented with permission of the Aging with Dignity organization, will be presented as a resource for the interviewing process. A sample case study is presented.
