A pocket calculator program for prediction of warfarin maintenance dose

A pocket calculator program for prediction of patient warfarin maintenance dose based on initial anticoagulant response is described. Four maintenance dose estimates are provided if a fixed initial dosing regimen (three 10 mg doses) is used and one estimate if any other regimen, which produces a prothrombin time ≥ 20 s, is used. Although all equations are based on reported literature data, their accuracy in large patient populations has not been fully established.
