Two patients experienced extensive complications related to local necrosis, one leading to gangrene requiring amputation, and the other developing chronic pain and hypersensitivity, with a permanent decrease range of motion. Upon admission to the intensive care unit, the patient was treated with calcium channel blockers IV and an intra-arterial injection of papaverine, which improved vessel dilation and flow to the hand. In a study by Sheth [4] at the University of Pittsburgh, Pittsburgh, Pa, the incident rate ratio for adverse drug events for promethazine was significantly higher (incident rate ratio, 4.32) compared with all other antiemetics combined. |
