Zusammenfassung:	Thachil et al detail a case of a 20-year-old woman who was admitted to the hospital because of fever, chest pain and hemoptysis. The case answers the question why wouldn't apparent iron-deficiency anemia improve with iron supplementation. Among others, the constellation of hemoptysis, reductions in forced expiratory volume and forced vital capacity, bilateral pulmonary infiltrates, and hemosiderin build-up in macrophages led to a diagnosis of idiopathic pulmonary hemosiderosis.
