Agile Implementation: a model to reduce the “Know-Do” gap in geriatric care

The Center for Health Innovation and Implementation Science at Indiana University School of Medicine developed the Agile Implementation (AI) Science Playbook, with the purpose of rapidly implementing evidence-based medicine into healthcare practices (Boustani et al., 2019). Through eliciting feedback from patients and colleagues, they established a clinic designed to care for PICS and other ICU-related conditions in the outpatient setting. In the study by Wang and colleagues, the longitudinal follow-up of caregivers did not report any improvement in caregiver stress.
