Zusammenfassung: | Five attributes characterize care delivered by a patient‐centered medical home (PCMH). Care is (1) comprehensive and team based including prevention, wellness, and chronic disease management; (2) patient centered and focused on relationship building; (3) coordinated across the healthcare continuum; (4) accessible via multiple modes of communication; and (5) committed to continuous quality improvement and patient safety.1 An evaluation by the National Committee for Quality Assurance revealed that PCMHs can reduce costs by decreasing hospital admissions and emergency department visits while improving patient outcomes.2 Our geriatrics clinic found that team training and safety culture assessment supported implementation and evaluation of PCMH processes. |
