Transplant Diabetes Program at Stanford Health Care: A Unique Peri-Transplant Diabetes Education Program

The Transplant Diabetes Program (Txp/DM Program) at Stanford Health Care (SHC) teaches diabetes mellitus (DM) management skills to pre- and post-, in- and outpatients adults within the kidney, liver, pancreas, heart, and lung transplant programs. We target patents demonstrating impaired glucose tolerance through casual plasma glucose levels >200 mg/dl or who have preexisting type 1 or 2 DM as described by Maskey. A1C is not recommended for screening due to low-test sensitivity in end stage renal disease. References
