Vitamin D deficiency is associated with digital ulcer but not with atherosclerosis or arterial stiffness in patients with systemic sclerosis: a pilot study

The objective of this study was to investigate the association of vitamin D deficiency with digital ulcers (DUs) that result from microvasculopathy, carotid intima-media thickness (CIMT) as surrogate markers of atherosclerosis, and brachial-ankle pulse wave velocity (baPWV) representing arterial sti...

In this cross-sectional study, 40 female SSc patients and 80 healthy controls matched for sex, age, and blood sampling season were recruited. Vitamin D deficiency was defined as serum 25-OHD levels &lt;30 ng/mL. “DUs ever” included active and healed DUs. CIMT and carotid plaque were examined using high-resolution ultrasonography, and baPWV was measured using an automatic waveform analyzer. Vitamin D deficiency was more prevalent in SSc patients than in controls (30 vs. 11.3%). Regarding SSc patients, 9 (22.5%) had DUs ever, and the mean CIMT and baPWV were 0.68 mm and 1561.1 cm/s, respectively; carotid plaque was detected in 13 (34.2%) patients. The frequency of DUs ever was significantly higher for SSc patients with vitamin D deficiency than those without this feature (50 vs. 10.7%, p  = 0.012), but the median CIMT and baPWV and the frequency of carotid plaque did not differ according to the presence of vitamin D deficiency. Multivariable logistic regression analysis showed that vitamin D deficiency was an independent risk factor for DUs ever (OR = 7.72, p  = 0.024). Vitamin D deficiency was associated with DUs, but not with atherosclerosis or arterial stiffness, potentially indicating that vitamin D may have different effects on the microvascular and macrovascular involvement in SSc pathophysiology.</description><identifier>ISSN: 0770-3198</identifier><identifier>EISSN: 1434-9949</identifier><identifier>DOI: 10.1007/s10067-017-3622-9</identifier><identifier>PMID: 28365806</identifier><language>eng</language><publisher>London: Springer London</publisher><subject>Aged ; Ankle ; Arteriosclerosis ; Atherosclerosis ; Atherosclerosis - blood ; Atherosclerosis - diagnostic imaging ; Atherosclerosis - etiology ; Carotid Intima-Media Thickness ; Case-Control Studies ; Cross-Sectional Studies ; Female ; Humans ; Medicine ; Medicine &amp; Public Health ; Microvasculature ; Middle Aged ; Original Article ; Pilot Projects ; Pulse Wave Analysis ; Rheumatology ; Scleroderma ; Scleroderma, Systemic - blood ; Scleroderma, Systemic - complications ; Scleroderma, Systemic - diagnostic imaging ; Skin Ulcer - blood ; Skin Ulcer - etiology ; Systemic sclerosis ; Ulcers ; Ultrasound ; Vascular Stiffness ; Velocity ; Vitamin D ; Vitamin D - analogs &amp; derivatives ; Vitamin D - blood ; Vitamin D Deficiency - complications ; Vitamin deficiency</subject><ispartof>Clinical rheumatology, 2017-06, Vol.36 (6), p.1325-1333</ispartof><rights>International League of Associations for Rheumatology (ILAR) 2017</rights><rights>Clinical Rheumatology is a copyright of Springer, 2017.</rights><lds50>peer_reviewed</lds50><woscitedreferencessubscribed>false</woscitedreferencessubscribed><citedby>FETCH-LOGICAL-c372t-7cda0a9adb317c1720711b6464e329e6d5e1b832581db6902b021eefb257078c3</citedby><cites>FETCH-LOGICAL-c372t-7cda0a9adb317c1720711b6464e329e6d5e1b832581db6902b021eefb257078c3</cites></display><links><openurl>$$Topenurl_article</openurl><openurlfulltext>$$Topenurlfull_article</openurlfulltext><thumbnail>$$Tsyndetics_thumb_exl</thumbnail><linktopdf>$$Uhttps://link.springer.com/content/pdf/10.1007/s10067-017-3622-9$$EPDF$$P50$$Gspringer$$H</linktopdf><linktohtml>$$Uhttps://link.springer.com/10.1007/s10067-017-3622-9$$EHTML$$P50$$Gspringer$$H</linktohtml><link.rule.ids>314,776,780,27901,27902,41464,42533,51294</link.rule.ids><backlink>$$Uhttps://www.ncbi.nlm.nih.gov/pubmed/28365806$$D View this record in MEDLINE/PubMed$$Hfree_for_read</backlink></links><search><creatorcontrib>Park, Eun-Kyoung</creatorcontrib><creatorcontrib>Park, Ji-Heh</creatorcontrib><creatorcontrib>Kweon, Seong-Min</creatorcontrib><creatorcontrib>Kim, Geun-Tae</creatorcontrib><creatorcontrib>Lee, Seung-Geun</creatorcontrib><title>Vitamin D deficiency is associated with digital ulcer but not with atherosclerosis or arterial stiffness in patients with systemic sclerosis: a pilot study</title><title>Clinical rheumatology</title><addtitle>Clin Rheumatol</addtitle><addtitle>Clin Rheumatol</addtitle><description>The objective of this study was to investigate the association of vitamin D deficiency with digital ulcers (DUs) that result from microvasculopathy, carotid intima-media thickness (CIMT) as surrogate markers of atherosclerosis, and brachial-ankle pulse wave velocity (baPWV) representing arterial stiffness in patients with systemic sclerosis (SSc).
