advocate cesarean delivery for all women with genital herpes simplex virus (HSV) lesions in labor to reduce vertical transmission. We believe it is time to revisit the practice of cesarean delivery for women with active, recurrent genital HSV infection. One in three babies are born through cesarean delivery in the United States, reflecting a 50% rise in cesarean rates in the past 20 years.
Approximately 85% of cases of neonatal HSV infection are contracted during delivery, with primary maternal HSV infection conferring the greatest risk.
