Zusammenfassung:As articulated by the Institute of Medicine and reinforced by the recent health reform law, the United States is fostering a deliberate shift toward a more population-based approach to promoting health. The opportunity has never been better and the demand has never been greater to identify and promote a key element of population-based health supporting individuals and their family caregivers in the self-management of chronic health conditions. Among other things Coleman shares that many physicians have come to recognize the weak link between providing patients with educational materials or care instructions and realizing the desired health behaviors.
