Hospital-at-home integrated care programme in older patients with orthopaedic conditions: early community reintegration maximising physical function

Highlights • Older citizens with orthopaedic conditions are at high risk of permanent disability. • Early community reinsertion with physical function maximisation must be facilitated. • We present an innovative programme for the care of vulnerable older patients at home. • Patients attended by the hospital-at-home unit had good functional outcomes. • Compared to usual care, hip fracture patients recover physical function sooner.
