Zusammenfassung:Chiarchiaro et al talk about traditional advance care planning which focuses on the completion of legal documents. In theory, these documents are easy to complete and disseminate. In practice, they have had limited effect for a variety of reasons. In response to these shortcomings, facilitated advance care planning has been developed in which trained facilitators serially engage with patients and families to aid patients in reflecting on goals and values, documenting their preferences, and sharing with their families.
