Zusammenfassung: | An abstract of the study by Meier et al describing the role of home-based palliative care in the continuum of palliative care programs, is presented. Describing the characteristics of diverse successful home-based palliative care models and explaining its challenges and opportunities in order to determine which characteristics best align with the community are mentioned. Also noted the replicable principles and practices shared by panelist of this session, from four pioneering home-based palliative care programs providing services to seriously ill patients in a variety of living situations. |
