Zusammenfassung:BackgroundLong term loss of work capacity is one of socially significant sequelae of any chronic condition, including ankylosing spondylitis (AS).ObjectivesTo analyze causes and predictors of long term loss of work disability in AS patients.MethodsThe study included 220 consecutive AS patients (162 males and 58 females), fulfilling criteria 1984 y., hospitalized during the period 2012-13 yy. Mean patients' age was 35,1±9,5 y, mean age at disease onset - 30,0±9,7 y. 92% of patients were HLA-B27-positive, disease activity by BASDAI - 4,6±2,1, ASDASCRP - 3,3±1,3. Functional impairment BASFI index was 4,1±2,6.Results56,4% patients out of 220 presented with long term loss of work disability. Mean age of patients with long term loss of work disability was 36,7±9,7 y. ROC-curve analysis indicated BASFI as most significant prognostic factor of long term loss of work disability. Threshold BASFI value of 4,7 allows to predict in an individual patient long term loss of work disability with (52,8%) sensitivity and (76,3%) specificity. AS duration was not a predictor of long term loss of work disability, although with longer AS duration the rate of long term loss of work disability was also increasing, i.e. from 32% by the end of the 2nd year to 63% - by the end of the 4th year.ConclusionsFunctional impairment with BASFI threshold of 4,7 allows to predict a long term loss of work disability in a particular patient with high sensitivity and specificity.Disclosure of InterestNone declared
