INCREASING CARDIAC REHABILITATION PROGRAM REFERRALS: A CLINIC-BASED INTERVENTION

From January 1 to June 30, 2016, a CR staff member liaison was physically present during clinic to a) pre-screen via electronic medical record for eligibility, b) speak to and schedule patients, and c) serve as a visible reminder to physicians to consider CR referral.
