A 77-year-old man was admitted to the hospital because of collapse.
There was a long history of obesity and heavy smoking. Twenty-one years before admission the patient had a myocardial infarct and retired from his work as a police officer. He subsequently experienced occasional episodes of chest pain, which was relieved by nitroglycerin. Three years before entry he began to have postprandial epigastric pain, nausea, and vomiting every three to four days.
