Zusammenfassung:IntroductionAdipocytokines affect numerous atherogenic processes including endothelial function, coagulation, and inflammation. Obese individuals have a proatherogenic adipocytokine profile. We measured the effects of increasing obesity on the adipocytokine profile. We hypothesize that increasing degrees of obesity are associated with a more atherogenic profile.Methods and ResultsWe evaluated 23 overweight or obese patients with type 2 diabetes. Labs were drawn at baseline and following a standard test meal. The data were analyzed using Student's paired t-tests, and ANOVA. Subjects were grouped by BMI into quartiles. Subjects in the highest BMI quartile had significantly higher PAI-1 and lower adiponection concentrations compared to subjects in the lowest BMI quartile. There was a trend of increased hsCRP and fibrinogen with increasing BMI quartiles.  Additionally, PAI-1 was elevated during the postprandial period in the highest BMI quartile compared to all other quartiles.ConclusionIncreasing quartiles of obesity lead to a more atherogenic, adipocytokine profile. Additionally, increasing adiposity blunts the normal diurnal decrease in PAI-1. These data suggest that atherogenic risk may increase in parallel to increases in adiposity.
