Methods In September 2012, a standardized HTN action plan was implemented for the process of checking a 2nd blood pressure (BP) if initial is abnormal (>=140/90), and scheduling a follow-up nurse or practitioner visit within 4 weeks when the 2nd BP is abnormal.
