Zusammenfassung: | A 48-year-old white woman presented with the chief complaint of worsening of facial hirsutism during a period of 6 months. Her history was remarkable for type 2 diabetes mellitus, hypertension, hypokalemia, and weight gain. All these conditions were diagnosed within approximately a 1-year period. Review of systems was remarkable for intermittent headaches and proximal muscle weakness. The patient is currently receiving corticosteroid replacement therapy. The failure to detect hormone-secreting pituitary microadenomas by MRI often results in invasive procedures, such as inferior petrosal sinus sampling (IPSS), to confirm the diagnosis. Dynamic contrast-enhanced MRI may eliminate the need for IPSS to confirm the diagnosis of ACTH-secreting pituitary microadenomas in some cases. |
