Zusammenfassung: | DISCUSSION: Pharmacists' participation in an interdisciplinary memory clinic represents a novel practice model that has hitherto not been described. PTPs were common in our patients, as shown by an average of 2 medication-related interventions. Enhancing the appropriateness of medication regimens is central to optimizing the care of vulnerable patients who are at high risk for adverse health outcomes (e.g., drug-induced cognitive and functional impairment). With proper training, pharmacists can readily provide care to complex patients with AD and related disorders. |
