Zusammenfassung: | The Doppler-index of total myocardial performance(Tei-index) is independent of blood pressure and heart rate.Deceleration time of wave A of mitral flow(DtA) is a new,promising,Doppler parameter to study diastolic dysfunction.We studied the influence of hypertension on Tei-index and DtA in patients with Acute Myocardial Infarction(AMI). We divided 100 patients(74 men,26 women,mean age 60.4±9.8 years) with first AMI(40 anterior,60 inferior) in two groups:A:54 patients without history of hypertension and B:46 patients with history of hypertension.Use of thrombolytics and ACE-inhibitors was comparable in both groups A and B.All patients underwent conventional Echo-Doppler study and measurement of Tei-index and DtA the 8th post-infarct day. Tei-index did not differ significantly between group A(0.651±0.026) and group B(0.682±0.035),while DtA was 98.15±2.69 in group A and 105.98±2.85 in group B (p=0.048). Tei-index did not differ between the two groups,thus means that hypertension does not affect the global performance of left ventricle in patients with AMI.DtA was significantly higher in group B, thus means that hypertension aggravates diastolic function in patients with AMI. |
