Zusammenfassung:Age>60 years and/or high cardiovascular risk profile are factors which define “non-optimal” potential organ donors (PD). In these PD a careful evaluation of renal damage, comprehensive of histological assessment of kidneys, has been performed to optimise organs utilisation for transplant. Aim of this study was to evaluate histological renal damage in a PD pool respect to their cardiovascular risk profile (CVRP) defined by age, cause of death, either cerebro-cardiovascular (CCV) or non-vascular (NV) accident, the presence of anamnestic high blood pressure (HBP) or left ventricular hypertrophy (LVH). Renal biopsies evaluation comprehended the determination of a kidney score (KS), derived from the assessment of glomerulosclerosis, tubular atrophy, interstitial fibrosis and vascular damage. The KS range was 0-12, the less KS, the less target organ. A KS>3 usually identify a kidney not utilised for single transplant and is therefore considered to identify renal damage. We collected biopsy from 94 PD (45 M, 49 F, mean age 66.2 yrs, range 41-80). 78 PD had CCV death (82.9%) and 16 NV (17.1%). HBP was referred in 52 PD (55.3%). ECG was available in 73 with LVH in 14 (19.1%). 41 PD (43.6%) had KS>3. The percentage of KS>3 respect to cardiovascular risk profile is represented in the table.
