P-529: The activation of systemic angiotensin converting enzyme as a new mechanism of hypertension in insulin resistance. Correlation with intima-media thickness

Introduction: The degree of Insulinemia or Insulin Resistance (IR), has been implicated in the genesis of hypertension through different mechanisms including the activation of histical Angiotensin Converting Enzyme (ACE) but not systemic/circulating ACE of pulmonary origin. Zusammenfassung:Introduction: The degree of Insulinemia or Insulin Resistance (IR), has been implicated in the genesis of hypertension through different mechanisms including the activation of histical Angiotensin Converting Enzyme (ACE) but not systemic/circulating ACE of pulmonary origin. Intima-Media Thickness (IMT) is an independent marker of cardiovascular risk related to atherosclerosis and left ventricular hypertrophy. Its relation with IR has been insufficiently studied. Aims: 1) Study the correlation between ACE levels and IR. 2) Verify 24h Blood Pressure, IR and ACE impact on IMT. Patients and Methods: Study Population: N=64 from a cardiovascular risk population, 36 F, 28 M, aged 45-80 (65 +/- 13), 15 type 2 diabetics (WHO criteria), 20 hypertensive (WHO criteria) and 20 dislipemic (LDLc >160 mg/dl). BP-Holter: SpaceLabs 90207. Four periods were evaluated: Global (24h), Daytime (9-22h), Nighttime (22-6h) and Critical (6-9h) periods. IMT: Carotid Doppler Ecography. HOMA: Fasting insulin (uU/mL) x fasting plasma glucose (mmol/L)/22.5. Statistical analysis: Linear regression. Results: 1) There is a positive linear correlation between HOMA and ACE levels (r=0,4; p
