A 68-year-old man was admitted with eosinophilic pustular folliculitis of Ofuji, a pruritic eruption caused by eosinophilic infiltration of the hair follicles. While he was an inpatient, a single, round (2 cm in diameter), tender, subepidermal nodule that was centrally elevated developed spontaneously overnight around a hair follicle on his right thigh. The lesion pigment changed from a skin-colored to an erythematous tone, flashing in time with the patient's pulse (Video 1), yet no single, subcutaneous arterial vessel could be palpated. Doppler ultrasonography revealed multiple superficial arterial and venous capillaries (Video 2), which were also evident on histologic examination (arrows). . . . |
