Zusammenfassung: | One cause for a failure is that the person is in a hurry to get the task completed. In this chapter, several examples are discussed in detail. One was an attempt to save time by using a pneumatic wrench to remove and replace the drain plug while changing the oil in a car. Another resulted from the desire of a company to quickly reap the direct labor savings that would accrue from the installation of an incentive system. Another resulted from the failure to read and fully comprehend the customer specifications since the company supplied a similar product to the same company in a different location. An extremely costly one occurred when the company converted a product without conducting a necessary mechanical testing. The converted product failed miserably resulting in the loss of thousands of dollars and the loss of prestige for this global corporation. |
