Zusammenfassung:This article presents a few observations on causes and prevention of failures in structural and equipment components that are related to the initiation and propagation of fatigue cracks. Some of these observations are illustrated by using actual failures. Also, the article presents a discussion of some common misconceptions and misinterpretations that have led to incorrect conclusions in failure investigations.
