만성 신질환 환자에서 발생한 Allopurinol에 의한 DRESS Syndrome

Drug Rash with Eosinophilia and Systemic Symptoms (DRESS) syndorme은 매우 드물고 과거에는 약물 과민증후군 (drug hypersensitivity syndrome)으로 명칭되었으며 호산구다증을 동반한 피부 발진, 발열, 림프절 비대 등을 특징으로 하고 일반적으로 약물치료 후 4-6주 이후에 발생한다1). Phenytoin, phenobarbital, carbamazepine 등의 항경련제나 설폰아마이드 계열의 약제들과 연관성이 높고, allopurinol 투여시에도 0.4% 정도로 발생한다고 보고되었으며 바이러스 감염과도 관련이 있다고 알려져 있다2). 고요산혈증은 만성 신질환의 악화 원인 중 하나이며, 경도에서 중등도의 만성 신질환이 있는 환자에서 저용량 allopurinol 투여는 안전하며, 단백뇨과 혈압을 감소시키므로 신기능의 보호 효과가 있다는 연구도 있다3). 저자들은 만성 신질환으로 치료 중이던 환자들에게서 고요산혈증으로 저용량 allopurinol을 복용한 후 전신의 피부발진 및 신기능의 악화를 동반한 전형적인 DRESS syndrome 4예를 경험하였기에 문헌고찰과 함께 보고한다. Allopurinol decreases serum uric acid levels in hyperuricemic patients and may slow the progression of renal diseases. Allopurinol-induced Drug Rash with Eosinophilia and Systemic Symptoms (DRESS) syndrome, which was called allopurinol hypersensitivity syndrome previously, is very rare and can be associated with serious complications characterized by fever, skin rash, acute hepatocelluar injury, renal insufficiency, leukocytosis, and eosinophilia. We report four cases of allopurinol- induced DRESS syndrome in patients with chronic kidney disease who had been taking allopurinol as a dose of 100 mg daily for 5 to 10 weeks. They presented with skin rash and eosinophilia. Blood chemistry showed acute aggravation of renal dysfunction. Allopurinol was discontinued immediately and treatment with glucocorticoid was started. All of the patients recovered from DRESS syndrome and renal function of them were returned to the basal levels in several weeks. (Korean J Nephrol 2006;25(4)814-687) KCI Citation Count: 0
