선천헝 수정체 결손증 환자에 적용된 인공수정체 광학부 포획 1예

목적 : 선천성 수정체 결손과 백내장이 함께 동반된 환자의 치료로 인공수정체 광학부 포획의 효과와 임상경과를 보고하고자 한다. 대상과 방법 : 5세 된 남아가 시력검사 위해 개인안과 방문 후 실시한 안과 검사에서 우안 수정체 결손과 백내장이 의심되어 정밀 검사위해 본원으로 전원되었다. 세극등 검사에서 우안 상이측부 수정체 결손과 수정체 중심과 가까운 결손부위에서 백내장이 관찰되어 수정체 제거후 후낭 원형 절개술을 시행하였으며 인공수정체의 지지부를 모양체 고랑 내에 위치시킨 후 후낭뒷편으로 광학부 포획을 실시하였다. 결과 : 술 전 최대교정시력 20/63에서 술 후 6개월째 최대교정시력 20/20으로 시력이 향상되었으며 인공수정체 중심이탈과 인공수정체 회전 및 시력저하를 야기하는 합병증은 관찰되지 않았다. 결론 : 선천성 수정체 결손의 치료로 인공수정체의 지지부를 모양체에 위치시키며 후낭 뒤편으로 광학부 포획을 실시하는 방법은 술 후 인공수정체의 안정성을 증가시키며 후낭혼탁을 예방하는 효과적이고 안정적인 방법으로 생각된다. Purpose: To report the effect of the optic capture in the treatment of congenital lens coloboma. Methods: A 5-year old boy was transferred to Maryknoll Hospital with congenital lens coloboma in his right eye. Lens coloboma at superotemporal area and capsular cataract around the coloboma was observed in the slit lamp examination. Posterior continuous curvillinear capsulorrhexis was performed after irrigation and aspiration of the lens. Optic was captured behind the posterior capsule while its haptics were placed in the sulcus. Results: Best corrected visual acuity improved from preoperative 20/63 to 20/20 postoperatively. Intraocular lens decentration and rotation and other sight threatening complications were not found during the follow up period. Conclusions: Optic capture is thought to be a safe and effective means to prevent IOL decentration and posterior capsular opacity in the treatment of congenital lens colobomas. KCI Citation Count: 0
