Sildenafil Citrate 복용 후 발생한 하직근 비대 1예

목적: Sildenafil citrate는 협심증 치료제로 개발되었으나 음경해면체의 phosphodiesterase (PDE)에 작용한다는 것이 밝혀지면서 발기부전 치료제로 널리 사용되는 약제이다. 본 저자는 sildenafil citrate 복용 이후에 발생한 하직근 비대 1예를 보고하고자 한다. 증례요약: 58세 남자 환자가 2주 전부터 시작된 양안복시를 주소로 내원하였다. 환자는 평소 기저 질환은 없던 자로, 2주 전 3회에걸쳐 sildenafil citrate를 복용한 바 있고 복용 2일 후에 우안 결막 충혈과 함께 양안 복시가 발생하였다고 하였다. 내원 당시 우안의하사시가 16 프리즘 디옵터(Prism diopter, PD)가 있었으며 상전 제한이 있었고 자기 공명 영상에서 우안 하직근 비대 및 조영증강이관찰되어 환자에게 경구 부신피질 호르몬 50 mg를 복용하도록 하였으며 2주 후 8PD로 감소하였다. 결론: 발기 부전 치료제로 널리 알려진 sildenafil citrate로 인한 외안근 비대는 보고된 바 없고 아직 그 기전에 대해 명확히 밝혀진바 없어 sildenafil citrate와 관련된 안과적 부작용 및 그 기전에 대한 더욱 많은 증례 보고 및 연구가 이루어져야 하겠다. Purpose: Sildenafil citrate was developed to treat angina, but was found to also inhibit PDE in the corpus callosum and is now widely used to treat impotence. We report a case of enlargement of the inferior rectus muscle after sildenafil citrate ingestion. Case summary: A 58-year-old male presented with binocular diplopia which started 2 weeks prior. He had no underlying disease. The patient had taken sildenafil citrate on 3 different occasions 2 weeks prior, and 2 days after his first ingestion,his right conjunctiva was injected and binocular diplopia started. On the first visit, he showed 16 PD hypotropia of the right eye with limitation of motion at upgaze. The MRI scan showed enlargement and enhancement of the inferior rectus muscle,and 50 mg of prednisone was prescribed. The amount of hypotropia decreased to 8 PD 2 weeks later. Conclusions: Enlargement of extraocular muscle caused by sildenafil citrate ingestion has not been reported in Korea, and related mechanisms are yet to be researched.
