타곤 근위 대퇴정을 이용한 대퇴골 전자간 골절의 치료

목적: 타곤 근위 대퇴정을 이용하여 대퇴골 전자간 골절을 치료한 후 임상적, 방사선학적 결과를 알아보고자 한다. 대상 및 방법: 2004년 8월부터 2010년 6월까지 타곤 근위 대퇴정을 이용하여 치료한 대퇴골 전자간 골절 155예를 대상으로 하였다. 남자가 54예, 여자가 101예 였으며 평균 연령은 74.1세 였다. 안정골절이 97예 그리고 불안정골절이 58예였다. 임상적으로 수술시간, 수혈빈도, 체중부하 시기, 수술 후 활동능력, 합병증을 조사하였고, 방사선학적으로 지연나사의 활강정도, 평균 골 유합시기를 조사하였다. 결과: 평균 수술시간은 64분이었고 수혈은 43예(27%)에서 시행하였다. 체중부하 시기는 평균 4.4일, 술 후 활동도는 96예(62%)에서 수상 전 상태로 회복되었다. 지연나사의 활강은 평균 5.6 mm, 유합시기는 평균 10주였다. 합병증은 총 6예(3.8%)로 회전방지 나사못의 근위부 돌출 3예, 금속정 주위골절 3예가 있었다. 결론: 대퇴골 전자간 골절에서 타곤 근위 대퇴정을 이용한 치료는 임상적, 방사선학적으로 우수한 결과를 보였으며 합병증 발생률이 낮았다. Purpose: To investigate the clinical and radiologic outcome after treatment of intertrochanteric fractures of femur using targon proximal femoral nails (PFN), Materials and Methods: One hundred fifty-five cases of intertrochanteric fracture who were treated with targon PFN from August 2004 to June 2010 were included, There were 54 men and 101 women with a mean age of 74.1 years at the time of surgery, There were 97 cases of stable fracture and 58 cases of unstable fracture. Operation time, frequency of transfusion, weight-bearing time, ability of activity after operation, and complications were all clinically investigated. Also, the average sliding of a lag screw and average union period were radiologically investigated. Results: The average operation time was 64 minutes and transfusion was required in 43 cases (27%). The average weight bearing time was 4.4 days. In 96 cases (62%), the pre-injury activity level was recovered. The average slide of the lag screw was 5.6 mm and the average union period was 10 weeks. There were 6 cases (3.8%) of complications including 3 cases antirotation pin cutout and 3 cases of periprosthetic fracture. Conclusion: The treatment of intertrochanteric fractures using targon PFNs showed a satisfactory clinical outcome and a lower radiological complication rate.
