수근 관절을 침범한 류마토이드 관절염에서 Sauve-Kapandji 술식과 병행한 수근 관절 유합술

목 적: 류마토이드 관절염 환자에게 시행한 Sauve-Kapandji 술식과 병행한 수근 관절 유합술의 임상적 결과를 평가하여 그 유용성을 알아보고자 하였다. 대상 및 방법: 1996년 2월부터 2003년 5월까지 Sauve-Kapandji 술식과 병행한 수근 관절 유합술을 시행 받은 류마토이드 관절염 환자 17명의 20 수근 관절을 대상으로 하였고, 평균 추시 기간은 42개월(범위, 13-100개월)이었다. 최종 추시 시 에 방사선학적, 기능적 결과를 평가하였는데 기능적 결과의 평가를 위해서는 Weiss 등에 의한 standard patient review questions와 Buck-Gramcko/Lohmann score를 이용하였다. 결 과: 임상적인 골 유합에는 약 6주가 소요되었고, 최종 추시 시에는 68°의 회내전과 71°의 회외전이 가능하였다. Weiss 등의 standard patient review questions에 의한 주관적인 기능 평가 점수는 10점 만점에 평균 8.25점(범위, 5-10점)이었고 17예(85%)가 수술 결과에 만족하였다. 16예(80%)는 수술적 치료를 좀 더 빨리 받는 것이 좋았을 것 같다고 응답하였고, 수술 후에 환측 상지의 최대한의 기능 회복에는 평균 6개월(범위, 1-30개월)이 소요되었다. 객관적인 기능 평가를 위한 Buck-Gramcko/Lohmann score는 10점 만점에 평균 8.7점(범위, 1-10점)으로 양호에 해당하였다. 결 론: 류마토이드 관절염이 요 수근 관절과 중수근 관절뿐만 아니라 원위 요척 관절까지 침범하여 심한 관절 파괴와 동통이 발생한 경우에는 Sauve-Kapandji 술식과 수근 관절 유합술을 동시에 시행하는 것이 기능을 보존하면서 증상을 경감시키고 변형을 방지하는 좋은 치료 방법이라고 생각된다. Purpose: To evaluate the functional results of total wrist arthrodesis with the Sauve-Kapandji procedure in the rheumatoid arthritis. Materials and Methods: The clinical results of twenty wrists of 17 patients who underwent total wrist arthrodesis with the Sauve-Kapandji procedure for the treatment of the rheumatoid arthritis during the period of February 1996 to May 2003 were analyzed. The patients who were observed for more than 12 months postoperatively were chosen as subjects and the mean follow-up period was 42 months (range, 13 to 100 months). At last follow-up, radiological and functional evaluation were done. For the functional evaluation, the standard patient review questions of Weiss et al. and the Buck-Gramcko/Lohmann score were used. Results: The mean time to clinical fusion of the wrist was 6 weeks. The postoperative range of motion is pronation 68°, supination 71°. The standard patient review questions of Weiss et al. revealed that 17 cases (85%) were satisfied with the functional results of the procedure. 16 cases (80%) wished that the proce-dure had been done earlier. The standard patient review questions also showed that maximum improvement in function after the procedure required an average of 6 months (range, 1 to 30 months). On a scale of 1 to 10, the mean score of the subjective overall functional evaluation by the patients was 8.25 points (range, 5-10 points).
