| Zusammenfassung: | We report a case of a ventral foramen magnum meningioma in a 73-year-old woman presenting with swallowing difficulty, posterior neck pain, gaiting difficulty, progressive motor and sensory deficits in all extemities, and intrinsic muscle atrophy in both hands. Magnetic resonace imaging revealed a huge mass, 4×4×4.5cm in size, located in anterior foramen magnum. We removed the mass by partial condylectomy via far lateral transcondylar approach without neurological deficit and postoperative craniovertebral instability.
