19세 여성에서 Chlamydia 감염에 의해 발생한 Fitz-Hugh-Curtis Syndrome 1예

Fitz-Hugh-Curtis syndrome (FHCS)은 여성 생식기 감염과 관련하여 간 피막에 염증(간 주위염)이 발생하는 것으로, 골반 내 감염 환자의 4∼14%에서 발생할 수 있는 것으로 알려져 있다. 우상복부에 날카로운 통증 또는 늑막염과 유사한 통증이 특징적이며, 항상 난관염의 증후가 동반되는 것은 아니기 때문에 담낭염이나 신우신염과 같은 질환과 감별이 필요하다. 정확한 발병 기전은 알려져 있지 않지만, Neisseria gonorrhoeae와 Chlamydia trachomatis가 주요 원인균으로 알려져 있다. 저자들은 최근 성경험이 있었던 19세 여성 환자에게서 Chlamydia 감염에 의해 FHCS가 발생된 증례를 경험하였기에 보고하는 바이다. Generally presenting as sharp, pleuritic right upper quadrant pain, usually but not always accompanied by signs of salpingitis. It needs differential diagnosis such as cholecystitis, pyelonephritis. The pathogenesis of FHCS is incompletely understood. But Neisseria gonorrhoeae and Chlamydia trachomatis are thought to be the primary causative germs of FHCS.
