안쪽눈구석에 발생한 유두상 땀관종

목적: 눈꺼풀에 매우 드물게 발생하는 유두상 땀관종을 보고하고자 한다. 증례요약: 74세 여성이 3개월 전부터 커지는 우안 안쪽눈구석의 종괴를 주소로 내원하였다. 종괴는 우안 눈꺼풀 내측에 중심부 지름 약 5.0 mm 크기로 궤양을 동반하였으며, 여러 개의 유두상 소엽으로 이루어져 있었다. 절제생검을 시행하였고 조직병리 검사상 종괴는 편평상피의 과다증식을 동반한 유두종 모양을 보이고 아래쪽으로 분화구 형태로 함입된 모양이 특징적이었다. 이는 두 개의 세포층으로 이루어져 유두모양의 낭종 형태를 보이는데, 바깥쪽은 입방형의 기저세포, 안쪽은 원주형태의 관강세포로 구성되어 있어 유두상 땀관종을 진단하였다. 결론: 눈꺼풀에 발생한 종괴에서 유두상 땀관종을 진단하였으며 유두상 땀관종은 그 임상 양상이 매우 다양하므로 눈꺼풀 종괴의 감별진단으로 이를 고려하여야 한다. Purpose: To report a rare case of syringocystadenoma papilliferum occurring in the medial canthal area. Case summary: A 74-year-old female presented with a growing mass on the nasal side of the right eye for the past 3 months. The mass, measuring approximately 5.0 mm in central diameter, was located on the inner aspect of the right eyelid and was ulcerated. An excisional biopsy was performed, and the histopathological examination revealed a lesion characterized by exophytic squamous epithelial hyperplasia with central invagination forming a crater-like structure. The lesion had a bilayer configuration, with cuboidal basal cells in the periphery and columnar luminal cells on the inside. These findings were consistent with the diagnosis of syringocystadenoma papilliferum. Conclusions: Syringocystadenoma papilliferum was diagnosed in an eyelid mass, emphasizing the need to consider this condition in the differential diagnosis of eyelid masses given the diverse clinical manifestations associated with syringocystadenoma papilliferum.
