거대낭종 소안구증 1예

목적: 출생 시부터 서서히 진행된 좌측 안구돌출을 주소로 내원한 생후 4개월 소안구 남아에서 시신경을 외측으로 미는 거대한 안와낭종을 제거한 사례를 보고하고자 한다. 증례요약: 생후 4개월 남아가 출생 이후 진행하는 좌안의 안구돌출을 주소로 내원하였다. 이학적 검사상 저명한 좌안 안구돌출 및자기공명영상검사상 2.2×1.9 cm 크기의 거대낭종에 동반된 소안구증 소견이 확인되었다. 좌안 앞쪽 안와절개술을 통해 낭종 박리와절제를 시행하였다. 조직병리검사상 신경외배엽 조직을 동반한 안와 낭종이 확인되었고 수술 후 재발 없이 미용상 양호한 소견을보여 경과 관찰 중이다. 결론: 안와 낭종이 동반된 소안구증에서 수술적 절제가 필수적인 치료는 아니나, 안면불균형과 각막노출 등 심한 안구돌출 소견을보이는 경우 안와 낭종을 제거하는 것이 추후 안구 크기 유지 및 기능적, 미용적 예후가 좋을 것으로 사료되어 안와 낭종제거술 시행후 양호한 경과를 보이는 사례를 보고한다. Purpose: We present the case of a 4-month-old boy with a large orbital cyst that caused lateral displacement of the optic nerve resulting in progressive exophthalmos of the left eye since birth. Case summary: A 4-month-old boy presented with progressive exophthalmos of the left eye since birth. Physical examination revealed marked left eye proptosis. Magnetic resonance imaging revealed microphthalmia and a 2.2 × 1.9 cm orbital cyst in the retrobulbar region. Subsequently, an anterior orbitotomy of the left eye was performed to dissect and excise the cyst. Histopathological examination revealed an orbital cyst containing neuroectodermal tissue. No complications or recurrences were observed during the 3-month post-surgery period. Conclusions: Congenital microphthalmia with orbital cysts is a rare condition that does not always require surgical intervention. However, timely surgical intervention in cases of severe exophthalmos can result in favorable functional and cosmetic outcomes.
