Zusammenfassung:「Abstract」: Background and objective: We report a case of comprehensive treatment for chronic periodontitis patient followed by Guided Tissue Regeneration (GTR) and dental implant. Finally, the patient had restoration of occlusal function and sensuousness. We present a therapeutic process of the patient from first visit to supportive periodontal therapy (SPT), and report a task of dental hygienist. Materials and Methods: On October 18 2007, the patient visited Nihon University Hospital at Matsudo for the first time. She was 64 years old and her chief complaint was increasing tooth mobility for several months. Results: There were symptoms such as gum redness and swelling, and plaque control record was 62%. Maxillary left first premolar had over 10 mm probing pocket depth (PD) and deep vertical bone loss. There was 7 mm PD and osseous defect mesial to a mandibular left first molar. We diagnosed her as localized advanced chronic periodontitis and occlusal trauma. During initial preparation we gave an explanation of treatment plan, and oral hygiene instruction, scaling and root planing, occlusal adjustment and tooth extraction. Thereafter, we performed GTR on mesial bone defect of mandibular left first molar. Finally, dental implant was placed in the extraction place of maxillary left first premolar. Thereafter, we treat her as SPT phase patient. Conclusion: In this case, we treated patient followed by GTR and dental implant. We suppose good communication during initial preparation built a highly-motivated patient, therefore we could do oral hygiene instruction smoothly and reduce an inflammation. We should pay attention to recurrence after the surgery and continue regular SPT.
