Zusammenfassung:Forel-H-tomy was originally designed by Dr. Jinnai to interrupt the descending impulses of convulsion at Fords H field of the subthalamus. It has been, thus, clinically applied mainly to the convulsive phenomenon of the epilepsy. Recently the authors found the following interesting results in epileptic phenomena other than convulsion. I. Pure petit mal seizures: Eleven years old Japanese boy (A. U.) had complained of pure petit mal seizures since 9 years old. The seizures consisted of the absence of consciousness lasting for 10 to 15 seconds, which corresponded with paroxysmal EEG outbursts, 3 c/s diffuse spike and wave complex. The seizures occurred at almost one hundred times a day under the medication consisting of Tegretol (0.4), Mysoline (0.4), Aleviatin (0.1), Diamox (0.25), Zarontin (0.75) and others. Since Forel-H-tomy was performed at the right side over 6 months ago, the following effects have been obtained under the mediation of Zarontin (0.2): 1) Diffuse SWC lasts only for few seconds, of which occurence decreases remarkably, 2) absence is clinically hardly noticed, and 3) in psychological standpoint, self-confidence and brighteness regain and mental test (WISC) revealed an increase in IQ from 77 to 102. II. Irritable behavioral disorders in epilepsy: Seven years old Japanese girl (H. S.) had complained of convulsion (left half of the body) and violent behavioral disorders under the medication of Ospolot (0.4), Aleviatin (0.15), Luminal (0.05), Tegretol (0.4), Wintamin (0.025) and others. Since Forel-H-tomy was performed at the right side over 11 months ago, remarkable improvements have been obtained under none of the medication. 1) Clinical seizures have disappeared and EEG abnormalities of the right hemisphere (suffuse dysrhythmia) have been also improved. 2) Preoperative character disorders such as aggressiveness, impulsiveness and unsteadiness disappeared. Mental test, which could be hardly performed before surgery because of her incorperation revealed an improvement of mental condition and also IQ after surgery.
