백내장 수술 후 스펙트럼영역 빛간섭단층촬영에서 나타나는 시신경 지표 변화

목적: 백내장 수술 전, 후 스펙트럼영역 빛간섭단층촬영(Spectral-domain Optical Coherence Tomography, OCT)에서 나타나는 시신경 관련 지표들이 변화하는 양상을 비교해 보고자 하였다. 대상과 방법: 백내장 수술을 받은 26명, 29안을 대상으로 하였다. 백내장 수술 전 후로 스펙트럼영역 빛간섭단층촬영을 이용, 시신경유 두 주위 망막신경섬유층 두께, 시신경유두 분석을 시행하였다. 신호강도, 사분면, 시구역 망막신경섬유층 두께, 테면적(rim area), 시신 경유두면적(disc area), 유두함몰면적비(cup / disc area ratio), 수직유두함몰비(vertical cup / disc ratio), 유두함몰부피(cup volume) 등의 자료를 비교분석하였다. 결과: 백내장 수술 후 신호강도, 망막신경섬유층 두께 모두 증가하는 것으로 조사되었다(p＜0.05). 시신경유두 관련 지표들 중 테면적은 0.07 ± 0.10 mm2 만큼 증가하였고, 시신경유두면적, 유두함몰면적비, 수직유두함몰비, 유두함몰부피는 0.07 ± 0.15 mm2, 0.04 ± 0.04, 0.03 ± 0.05, 0.04 ± 0.06 mm3 만큼 감소하였다(p＜0.05). 결론: 백내장이 있는 경우 빛간섭단층촬영상 나타나는 수술 전 시신경유두 주위 망막신경섬유층 두께와 신호강도를 낮게 만들었고 다른 시신경유두 지표들도 통계적으로 유의한 변화를 나타내어 결과 해석할 경우 고려되어야 한다. Purpose: To assess changes in peripapillary retinal nerve fiber layer (RNFL) thickness and optic nerve head parameters after cataract surgery by using spectral-domain optical coherence tomography (OCT). Methods: Twenty-nine eyes of 26 patients, who underwent cataract surgery, were imaged with spectral-domain OCT before and after surgery to measure peripapillary RNFL thickness and optic nerve head parameters, signal strength (SS), quadrant, 12 clock-hour RNFL thickness, rim area, disc area, cup/disc area ratio, vertical cup/disc ratio, and cup volume. Results: The postoperative RNFL thickness and SS were higher than before surgery (p < 0.05). Regarding optic nerve head parameters, rim area was 0.07 ± 0.10 mm2 higher than before surgery and disc area, cup/disc area ratio, vertical cup/disc ratio, cup volume were 0.07 ± 0.15 mm2, 0.04 ± 0.04, 0.03 ± 0.05, 0.04 ± 0.06 mm3, respectively, lower than before surgery (p < 0.05). Conclusions: Cataracts may decrease peripapillary RNFL thickness measurement and SS on OCT scans and change other optic nerve head parameters. Peripapillary RNFL thickness and optic nerve head parameter measurements should be interpreted with caution in eyes with significant cataracts. J Korean Ophthalmol Soc 2013;54(10):1573-1580
